This COVID-19 Health Declaration Form is intended to govern the access to the United Nations Conference Centre, Bangkok (UNCC) to keep it safe for all persons. All on-site Secretariat Staff Members are required to complete this COVID-19 Health Declaration Form in order to receive the meeting badges.

Upon submission of your COVID-19 Health Declaration Form, you will receive a clearance email, which you need to show at the UNCC Registration Desk to get your meeting badge issued. All Staff Members shall collect their meeting badge at UNCC registration counter on the Ground floor, from 09 to 20 May 2022.
