All on-site Meeting Participants and Staff Members are required to complete this COVID-19 Health Declaration Form in order to receive the meeting badges.

Upon submission of your COVID-19 Health Declaration Form, you will receive a clearance email, which you need to show at the UNCC Registration Desk to get your meeting badge issued.
