This COVID-19 Health Declaration Form is intended to govern the access to the meeting venue to keep it safe for all persons. All on-site Meeting Participants and Staff Members are required to complete this COVID-19 Health Declaration Form in order to receive the meeting badges.
Upon submission of your COVID-19 Health Declaration Form, you will receive a clearance email, which you need to show at the Registration Desk to get your meeting badge issued.
ATTENTION: For Non-Governmental Organizations, in the event that your organization is not listed under "Organization Name", please change your "Representation Type" to "Non-ESCAP Meeting Participants" and type your organization name. Thank you!
